Before You Apply

By completing and submitting this application, you:

  1. Consent to the disclosure of the information provided in this application.
  2. Acknowledge the authority of Hockey Canada, BC Hockey, PCAHA and Surrey Minor Hockey Association and agree to carry out and abide by their constitutions, bylaws, rules and regulations.
  3. Confirm that you have read and understand the coach's role as outlined in the SMHA Coaches Code of Conduct.
  4. Agree to familiarize yourself with the National Coaching Certification Program (NCCP) requirements for coaching minor hockey and maintain the required level of certification.
  5. Agree to provide a Criminal Record Check prior to the start of the season.
